HOME VISITING SERVICES FOR CHILDREN

IDCC also facilitates a specialized home-visiting program for children who cannot regularly attend necessary care visits. We will provide care managers who help children and their families, acting as an advocate for their various needs.
Managers will orchestrate a care team that may include a therapist, psychiatrist, para-professional, teacher and or counselor to ensure the child’s health needs are taken care of. Additionally, they can help coordinate government programs such as food stamps and provide educational information concerning the child’s diagnosis.

Our specialized home-visiting program is designed to bridge this gap by bringing essential care directly to the child and their family’s doorstep. TO QUALIFY FOR IDCC HOME VISITING PROGRAM YOU NEED TO HAVE:

Children must have Medicaid or Managed Care Medicaid. Adults must have Managed Care Medicaid. Those with Dual Medicaid/Medicare, Medicare only or Commercial insurances will not be accepted.
