Care Management

At IDCC, we work carefully with individuals of all backgrounds in our Care Management program, one of our organization’s hallmark services. IDCC Care Management takes the case management process to a whole new level. Our professional care managers are trained to support each individual’s recovery through a holistic approach that considers each person’s overall mental, physical and social needs. In our program, care managers not only organize traditional mental health services, but also act as a resource to coordinate primary health care, housing, transportation, employment, social relationships, and community participation. From accompanying trips to the mall for clothing to teaching proper nutrition and personal budgeting practices, our care managers provide many services that support the determined plan of care and help individuals obtain their goals.

Children’s Care Management Services

This program provides supportive care management services to mentally, behaviorally, or emotionally challenged youth throughout Brooklyn. Through home visits, families are provided with ongoing assistance and advocacy to access mental health, education and income support programs that are essential to meeting their child’s needs. Our care managers have been trained to work with children and their family, ensuring services are tailored to best address their strengths and needs. Services can be short- or long-term, depending on the needs of the family.
